Our goal is to assess the possible role of online medical consultations among the clients of homeless care institutions. The uniqueness of our initiative is that the participants of the project can receive health services within the social institution with the help of appropriate telecare assistance, so the efficiency and continuity of chronic care might be significantly increased, and the digital skills of our clients can also be developed.
Based on our previous experience, we are creating a holistic, person-centered healthcare program in which the individual primary care services (health center open 24/7, mobile medical outreach services, online visits) synergize with each other to help improve access and quality care.
In our research, we examine our homeless clients’ access to digital devices, their use of the Internet for health purposes, and their digital health literacy. A better understanding of the habits and skills of people who are already digitally active can form the basis of an upcoming educational program and the introduction of remote patient monitoring technologies.
We believe that the long-term health status of our patients with chronic conditions can be greatly improved with screenings. Undiagnosed and untreated lower extremity arterial disease in an advanced state often leads to amputation, which significantly worsens the quality of life and life prospects of homeless people. With our screening test, we aim at early detection and non-surgical treatment of the problem.
We are open to learning about our clients’ expectations and their feedback on our work. By taking individual needs into account, we can provide more personalized care, can create a better doctor-patient relationship, and an atmosphere of trust always contributes to healing and a higher willingness to cooperate.
